How do I submit a Disability claim?

You can file a Short Term Disability (STD) claim either online or by completing a paper claim form.

Online process

When filing a disability claim online, you must complete the online form and provide authorization for your provider to release medical information to Guardian.

Note: Claims can be submitted after the disability begins or up to 14 days before the disability begins.

1. Log in to Guardian Anytime. For details, refer to: As a member, how do I register for Guardian Anytime?

2. From the menu, select one of the following. Not all plans have all options.

   • Click Absences then My Leave then Plan an Absence then Leave of absence.

   • Click Absences then Absences dashboard then Plan an Absence then Leave of absence.

   • Click Claims then Submit a claim then Short Term Disability.

3. Complete the online form. If the form doesn't appear, follow the instructions below for the Paper claim process.

4. After submitting the claim, you must provide authorization for Guardian to contact your provider.

   1. Complete the Authorization to Obtain Information form.

   2. Give the form to your provider.

   3. Scan and upload the form using the Guardian Anytime Secure Channel link.

Paper claim process

When filing a Disability claim on paper, you must submit the claim form to Guardian and provide authorization for your provider to release medical information to Guardian.

1. Click the appropriate link in the chart below to access and print the claim form.

2. Complete the claim form.

3. Complete the Authorization to Obtain Information form.

4. Submit both the claim and the authorization form by one of the following methods.

   Paper Claim Submission Methods
