Indicate any restrictions or limitations and verify your employer will accommodate them.
Returning part-time
If you are returning to work on a part-time basis, partial disability benefits may apply.
Return to work is delayed
If your physician delays your return to work date, the physician must complete a medical update. Please contact Guardian at the number above to inquire about the medical information required for an extension.
