Can I set up a direct deposit for Guardian claim payments?

Guardian offers an option for direct deposit of claim payments for Short-Term Disability (STD) and Long-Term Disability (LTD).

Follow these steps to set up direct deposit. 

For STD: 

• Download the Direct Pay Enrollment-STD form available online. 
• Once you're done, please submit the form along with a voided check, bank deposit slip using one of the following methods.
• Online - Upload using the Secure Channel link on Guardian Anytime. If you want to learn more, you can read the article.
• Email - [e-mail]
• Mail or Fax - Send to the address or fax number provided on the form.

For: LTD: 

• Download the Direct Pay Enrollment and Authorization Form available online. 
• Once you're done, please submit the form along with a voided check, bank deposit slip using one of the following methods.
• Online - Upload using the Secure Channel link on Guardian Anytime. If you would like to learn more, you can read the article.

• Mail or Fax - Send to the address or fax number provided on the form.

Generally, it takes approximately seven business days for direct deposit to be activated once received.
