You can access and submit various forms, including claim forms and evidence of insurability (EOI) forms, online through Guardian Anytime.
- Log in to Guardian Anytime.
- If you don't have a Guardian Anytime account, this article will provide the steps to create an account.
- Roll over the heading "Connect with us" to display a dropdown menu in the top banner.
- Select Forms & Claims.
- A blank field with a dropdown menu will display. Select the option "Benefits through work" to continue.
- Find a Form, Submit a Claim, Send a Document or Log in to Guardian Anytime will display.
- Choose the option that matches what you'd like to do.
