How do I submit a Guardian Evidence of Insurability (EOI) form?

When an employee requests coverage that requires additional underwriting, Guardian will inform the employee of the need to complete Evidence of Insurability (EOI) and provide instructions for online submission. Guardian's online EOI forms are a quicker, more secure alternative to traditional paper forms. 

To submit an online EOI form, follow the steps below. 

1. You will receive an email, or letter if an email is not available, from Guardian with instructions and a unique link to submit your EOI form online.

2. Then, you will register and create an account on Guardian Anytime. Then, simply fill out the form, electronically sign it, and click "Submit."

3. Once Guardian receives the form, the employee will receive an immediate status update. Additional updates will be sent from Underwriting as needed, including the final decision.    

In the event your requested coverage amount is not approved or you do not complete the necessary EOI form, your coverage amount will default to the guarantee-issue (GI) amount.
