Members can access Dental ID cards on Guardian Anytime. Follow the steps below to view, print, or order ID cards.
- Visit Guardian Anytime Login to access your Member account.
- From the menu options at the top of the page, roll over "Forms & materials" and then click "Obtain forms and materials."
- Select the hyperlink "Dental, Vision ID Cards."
Note: Only the registered member can view, print, or order ID cards. Dependents are not able to view, print, or order ID cards.
View or Print an ID Card
Click the View/Print button for the Dental ID card to view or print an ID card. A PDF version of the ID card will display. You can download the electronic copy or print a hard copy if preferred.
Note: If the card does not open in a new window, you may need to disable the pop-up blocker.
Order an ID Card
To order an ID card, click the Order check box for the ID cards you want to order.
Note: Physical ID cards are unavailable if the Order check box does not display.
- Answer the question, "Should Guardian ship to this address?".
- To send the card order to the address on file, click Yes.
- Click No to ship the ID card to an alternate address and enter the new shipping address.
- Click the Add to order button.
- When the Order ID Cards Summary screen displays, select Place Order.
Note: Guardian sends Physical ID cards by USPS standard mail in a plain, unmarked envelope. Allow 7 - 10 business days for receipt.
